Safety Alert: Insulin

Recall of Single Lot of ReliOn Syringes

What does this mean?

Since using these mislabeled syringes may lead to an overdose of insulin and serious health problems, this recall should be taken seriously. The ReliOn syringes are sold only at Wal-Mart stores and Sam's Clubs. Look at your syringes to see if they are ReliOn syringes and find the "lot" number printed on the box or packaging. If your syringes are ReliOn syringes and have a lot number that matches lot number 813900, return all of these syringes to your local Wal-Mart store or Sam's Club pharmacy to examine and possibly exchange. If you are not sure who has made your syringes, or cannot find a "lot" number, please take your syringes and packaging to your pharmacist for them to check as soon as possible.

Tyco Healthcare Group LP (Covidien) is recalling one lot of ReliOn sterile, single-use, disposable, hypodermic syringes due to possible mislabeling. The use of these syringes may lead to patients receiving an overdose of insulin, which may lead to hypoglycemia (low blood glucose) and serious health problems. This recall applies to ReliOn 1cc, 31-gauge syringes lot number 813900. Only ReliOn syringes from this lot number and labeled as 100 units for use with U-100 insulin are the subject of the recall.
