After 15 years of glyburide/metformin, my pancreas said, "enough!!" and pretty much stopped producing insulin; so my endocrinologist started me on one 12-unit injection of Levemir daily at evenings, for basal maintenance, and Novolog, usually anywhere from 8-13 units ten minutes before eating, depending, of course, on my estimate (and boy, do you learn to "estimate" those carbs in a hurry, after a few hypos happen...) and my A1Cs went from 8.5-9.0 , in just a six-month period (some five years ago) to the mid-sixes. And last month, and a year ago, both at my annual physical exam, my A1Cs were 6.1 (last year) and 5.9 (May of 2010). Lots of finger-sticking and monitoring and calculating, and 4-6 injections per day, but well worth it! And usually, completely painless, with the teeny pen needles now available. I just returned from a trip to Africa (three weeks) and, given that the Novolog and the Levimir will both store, unrefrigerated, for up to 30-40 days, I only needed one Levemir pen and three Novolog pens for the whole trip ( I took along an extra Novolog pen, stored it separately from my other insulins....). Great medical technology, probably only "bested" by a pump, but I don't want to deal with the indwelling catheter set.

I am not sure what the person who said they use insulin on a "sliding scale" means, as I've heard it used different ways by different people. If you read "Using Insulin" by John Walsh, it can help figure out the relationship between the carbs you eat and the amount of insulin you need. I haven't noticed much correlation between my levoxyl and my novolog use, but I was already using levoxyl at a steady dose when I was diagnosed with diabetes (both thyroid & diabetes autoimmune forms)
