This recall for Glyburide and Metformin 1.25 mg /250 mg, 2.5 mg/500 mg and 5 mg/500 mg tablets from CorePharma, LLC was issued at a warehouse/pharmacy level only. The FDA and the manufacturer have determined that any recalled product that may have been dispensed to patients does not pose a significant health risk. CorePharma, LLC is recalling several lots of Glyburide and Metformin 1.25 mg /250 mg, 2.5 mg/500 mg and 5 mg/500 mg tablets because it is possible that the tablets contain tiny pieces of metal.

The following lot numbers have been recalled: 1) Glyburide and Metformin Tablets 1.25 mg/250 mg - Lot numbers CPI869, CPK819A, CPB968A, 100980, 101423B, CPK819B, CPB968B, and 101423A

2) Glyburide and Metformin Tablets 2.5 mg/500 mg - Lot numbers CPI812, CPK818 10/10, CPK829, CPA961, CPA962, CPB939, CPB940, CPB941, CPB955, CPB956, 100278, 100279, 100280, 100657, 100658, 100925, 100926, 101098, 101099, 101100, 101419, 101420, 101931, CPK816, CPK817, CPK830, CPL801, and CPB942

3) Glyburide and Metformin Tablets 5 mg/500 mg - Lot numbers CPK821C, 100281, 101564, CPK832B, CPL802B, CPK822, CPK823, CPK832, CPL802, CPL803, and CPL804

Glyburide and Metformin tablets are used in the treatment of diabetes. The combination of Glyburide and Metformin is also sold under the brand name Glucovance. This recall alert only applies to the select lots of the generic Glyburide and Metformin tablets made by CorePharma, LLC. No other lots, strengths, or brands were involved in this recall.
